Prior to proceeding with your application, the following terms must be accepted.

I warrant that the information given by me in this application is true in all respects. Furthermore, I understand and agree that any false statement on this application, or any other form utilized by the company will subject me to dismissal without notice. I hereby authorize you, my former employers, and references to furnish any information concerning my personal character, habits, or employment records and I hereby release all such persons from liability on account of having furnished this information. Furthermore, I also understand that I have the right to make a written request in a reasonable period of time to receive information about the nature and scope of any investigative consumer report if such is made. I agree not to infringe upon Stuller, Inc., or subsidiaries interests to any patents, trademarks or other trade secrets which may be held by them, including but not limited to, plans, processes, tools, mechanisms, customer information, compounds and formulas or intellectual property. I further agree to keep secret and take reasonable precautions to protect confidential information made known to me in the course of my employment.

I understand and agree than any associate handbook which I may receive will not constitute an employment contract, but will be a statement of general practices and procedures.

We maintain a drug-free workplace and perform pre-employment substance abuse testing. I understand that the possession and/or use of alcohol, illegal drugs, and non-prescribed controlled medications, dangerous substances, firearms, or stolen property is prohibited on any work location of Stuller and its subsidiaries, and that violation of this policy may result in disciplinary action up to and including immediate termination.

If I am involved in an accident on Stuller property resulting in injury to an employee, property damage, or financial loss, I give my consent to a mandatory drug test as required under company policy. Any associate who, as a result of testing, is found to have any indentifiable traces of a prohibited drug or substance in their system, will be considered in violation of company policy and will be subject to disciplinary action up to and including termination.

I understand that all work areas are subject to electronic monitoring and unannounced searches for security reasons.

I hereby give my consent to Stuller, Inc., and its subsidiaries or its authorized representative to search my person and personal effects, to submit a drug and alcohol screen, or to submit to blood, urine, or hair samples for testing for determining compliance with this policy.

I agree to comply with Stuller's safety, security, and work rules.
